研究方向

肝细胞癌是我国第二位常见肿瘤,其病因学预防是本研究组的研究重点。我们将继续进行有半数新生儿接受了乙肝疫苗接种的,有8万人规模的,同龄人随机对照(因80年代乙肝疫苗来源有限),并可长期随访的肝癌高发农村的临床队列研究。目前已证实,抗乙肝免疫极显著地降低了与肝癌发生密切相关的慢性乙肝的患病率。预期在未来的10年内将见到肝细胞癌发生率及死亡率的显著下降。它将严格证明人类可用免疫接种来有效预防一个常见恶性肿瘤的同时,也充分证明乙肝病毒感染是肝癌的重要病因。此外我们还对肝癌的两个重要而又有协同效应的因素:乙肝病毒感染和黄曲霉素摄入同时下降对肝癌发生的影响进行了观察。其中P53基因249密码子的高突变率可能有重要意义。

Research Description

Hepatocellular carcinoma (HCC) ranks second in cancer incidence list of China. We are continuing the large scale, cluster randomized (on the basis of limited supply of vaccines in 1980th), controlled clinical trial on universal neonatal HB vaccination in an endemic rural population with the aim of significant protection at the morbidity of early chronic hepatitis B as well as at the incidence and mortality rate of HCC as the final goal. This is a long term collaborative studies of Chinese Academy of Med Sciences, Qidong Liver Cancer Institute and Oxford University. Serological and clinical follow-up has demonstrated that the HB vaccination has very significantly reduced the morbidity rate of chronic HB in children at 5 and 10 years of age. The reducing trend of HCC incidence in local adolescent populations in recent decades could be associated with the reduction of aflatoxin exposure in conjunction with the HB vaccination effect. 249 codon mutation of p53 gene in HCC might provide unique information to decode the multi-factorial hepatocarcinogenesis and its effective control pathways
